How To Receive

Details on how to apply

  1. Adult day health care requires a provider assessment.

  2. Following assessment, obtain a provider order for adult day health care.

  3. Contact the VNS Health MLTC Care Team at (888) 867-6555 (TTY: 711) for prior authorization and guidance.

  4. The Care Team will coordinate these services and ensure that all the care received is carefully coordinated in accordance with the Person-Centered Service Plan.

Adult daycare centers are designed to provide care and companionship for older adults who require assistance or supervision during the day, allowing seniors to socialize and enjoy planned activities in a group setting, while still receiving needed health services such as medical, nursing, dental, pharmaceutical (medication administration and management), nutritional, occupational therapy, social services, psychosocial assessment, rehabilitation, socialization, coordination of outpatient referrals, and leisure time and activities.

Adult day care centers vary in their programs and services, though typically offer intensive medical services and physical, occupational, and speech therapy, as well as social services for individuals with serious medical conditions and those at risk of requiring nursing home care. These centers provide frail adults experiencing physical, mental, or social problems associated with stroke, isolation, confusion, or other conditions with the opportunity to meet new people, participate in activities, and enjoy daily life in a comfortable, supervised setting.

Caregivers can also receive services at adult day care through counseling, support groups, and education/training courses.

Adult daycare centers are always staffed with a registered nurse and, commonly, several other health professionals. It is common practice for the program to require a health assessment by a physician prior to admission.

The Care Manager can arrange for your loved one to receive adult day health care in a residential health care facility or state-approved site supervised by a physician.

To participate in adult health day care, your loved one cannot be homebound and must require certain preventive or therapeutic services to attend.
