How To Receive
Details on how to apply
Supplies
Obtain a provider order for diabetes equipment and/or supplies.
Present it to a Medicare-certified provider, pharmacy, or supplier for fulfillment.
Shoes
Obtain a provider order for therapeutic shoes and a referral to a Medicare-certified diabetic shoe supplier.
Present the order to the supplier for fulfillment.

Coverage includes meters and continuous glucose monitors (CGM) that measure blood glucose (blood sugar) and related supplies, including test strips, lancets, lancet holders, and control solutions.
Tubing, insertion sets, and insulin for people using insulin pumps, and sensors, transmitters, and receivers for people using CGM are also covered.
In addition, one pair of extra-depth or custom shoes and inserts per year for people with specific diabetes-related foot problems are covered.
Note that Part D may cover injectable insulin, certain medical supplies used to inject insulin (syringes, test strips, alcohol swabs), disposable pumps, and some oral diabetes drugs. Check with your loved one's plan for more information. The cost of a one-month supply of insulin is capped at $35. Similar caps on costs apply for traditional insulin used in insulin pumps covered under Part B.
