How To Receive

Details on how to apply

  1. To find out more about CDPAS and determine if it is right for your loved one, contact the VNS Health MLTC Care Team at (888) 867-6555 (TTY: 711) to discuss.

  2. Obtain a network provider assessment and order for CDPAS.

  3. Follow the guidance provided by the Care Team.

    • Locate a network CDPAS: Click on the Home Care tab on the page, then click on Consumer Directed Personal Assistance Service (CDPAS).

    • Note that the Care Team will help you and your loved one connect with a fiscal intermediary (FI).

    • Family or friends can serve as personal assistance for the CDPAS program, except for the CDPAS designated representative. That individual may not act as a personal assistant.

  4. If your loved one is self-directing their personal care and the care provider, whether a family member or someone outside the family, is late or does not show up, this is a personal matter that must be discussed directly with the person providing care.

For most members, the home health aide (HHA) or personal care aide is the person who comes to their house regularly to ensure that day-to-day needs are met.

Members who need personal care services, home health aide services, or skilled nursing tasks may request that they be provided by a consumer-directed personal care assistant under their direction or the direction of a designated representative in the event the member is not self-directing). That person will be in charge of finding, hiring, training, scheduling, supervising and, if needed, terminating the personal assistant(s).

The CDPAS designated representative may not act as the personal assistant.

An Assessment Nurse will complete an assessment to determine your eligibility for CDPAS. The assessment determines the level of eligible assistance with personal care services, home health aide services, and/or skilled nursing services.

A fiscal intermediary (FI) manages the personal assistants’ wages and benefits. The FI will conduct background checks and drug tests to make sure assistants are able to work. They manage employment documents, time sheets, and annual worker health assessments. They don’t oversee any medical or personal care. The member or designated representative is in charge of this.

Once the FI reviews and approves the assistants’ work status, they can begin work.
