How To Receive

Details on how to apply

Before calling the VNS Health MLTC Care Team, complete these easy steps:

  1. Have the provider fax the Prosthetic or Orthotic request to the health plan at 212-897-9448, with the following:

    • Date

    • Member's name and date of birth

    • Diagnosis and related ICD-10 codes

    • Provider NPI number

    • Requested DME/supply products

    • Length of need and number of refills

    • Provider signature and date

  2. The following information is required to process the request. Please have this information ready before reaching out to the VNS Health MLTC Care Team.

    • Member name and ID number

    • Member height and weight

    • Type of item

    • Quantity of item

    • Frequency needed

    • Brand

    • Size

    • Name of vendor/provider

  3. The VNS Health MLTC Care Team coordinates these services.

You do not need to worry about locating and selecting a supplier in your area. VNS Health makes that determination for you and will contact the chosen supplier. The VNS Health MLTC Care Team is available to support you. Any member of the Care Team can research and respond to inquiries and questions regarding DME/Supplies. Issues will be escalated to your Care Manager if needed.

Prosthetic devices are devices (other than dental) that replace all or part of a body part or its function. While most people think of missing limb prosthetics, devices like pacemakers are also considered prosthetics.

Orthotic devices support a weak or deformed body part (as with special shoe or heel inserts) and restrict or eliminate motion in a diseased or injured part of the body (such as with a knee, back, or neck brace).
