Details on how to apply

  1. A network provider referral can be helpful if services are needed that are outside the Person-Centered Service Plan.

  2. Contact the VNS Health MLTC Care Team at (888) 867-6555 (TTY 711) for prior authorization.

    • The VNS Health MLTC Care Team coordinates these services, and they ensure that all the care received is carefully coordinated in accordance with the Person-Centered Service Plan.

Environmental factors in the home are those things in the home that typically assist with daily living. However, some environmental factors in the home can become barriers that reduce access and potentially create danger for someone with chronic conditions and functional limitations - things like stairs, low toilets and high tubs with no supports, poor lighting, poor visual contrast, lack of space, and infestations.

Social factors in the home are those things that help maintain social connection and interaction - things like means of communication, visits from family and friends, pets, entertainment (reading materials, TV, radio, computer, smartphone), and housekeeping and maintenance. When social factors are limited because of chronic conditions and a decline in mobility and function, it can create experiences of isolation, loneliness, depression, anxiety, unimportance, loss of purpose, and low self-esteem.

If needed, social and environmental items and services that support your loved one's medical needs and are included in the Person-Centered Service Plan can be coordinated and provided. These services and items include, but are not limited to, the following:
