How To Receive

Details on how to apply

  1. Schedule a network primary care provider appointment: Click on the Primary Care Providers tab on the page.

  2. Inform the network provider that it is for the “Welcome to Medicare” visit when scheduling.

  3. Attend the appointment.

  4. Engage in recommended follow-up treatment and/or referrals.

  5. If transportation is needed to and from the appointment:

    • Call (877) 718-4219 (TTY: 711), M-F 8am-8pm, at least 48 hours in advance of the needed transportation.

    • Have the date, time, location, and reason for transportation ready when calling.

    • Receive authorization and guidance on scheduling the transportation service.

    • Get confirmation of scheduled transportation.

    • Use the transportation service to and from the appointment.

This one-time visit occurs when someone initially signs up for Medicare/Medicare Advantage. This preventive visit must be scheduled within the first 12 months of having Medicare Part B (the medical insurance part).

When making the appointment, let the provider know it is for the “Welcome to Medicare” preventive visit and bring the following to the appointment:

If the provider performs additional tests or services during the same visit that Medicare doesn't cover under this preventive benefit, your loved one may have to pay coinsurance and the Part B deductible may apply. If Medicare doesn't cover the additional tests or services (like a routine physical exam), your loved one may have to pay the full amount.
