  1. Be sure to get an itemized invoice of services from the providers and facilities that provided care. The facility billing department can assist you.

  2. Complete and submit an electronic claim at the VNS Health website, or complete and submit a hard copy claim to mail to VNS Health.

  3. Use the itemized bill or invoice, along with the information above, to complete and submit a claim.
